Is it possible to use GEM library in vanilla?

If so, how do I install?

@cuinjune I don't know about your platform, but on linux it's as simple as:

• copy Gem.pd_linux to ~/pd-externals
• start pd with -path ~/pd-externals -lib Gem

How about on Mac OS X?
Could anyone tell me how to install on Mac?

Can't you simply reproduce the 2 steps above, but on Mac?

• download pd-extended for Mac
• show packages content
• find Gem.dynlib (I think on Mac the externals extensions are dynlib -- not sure)
• copy this file somewhere and specify the -path ... / -lib Gem when launching pd (from gui or command line).

compiling Gem in OS X is pretty easy. There are instructions with the library, can't really remember now. I just checked the readme and it says that you should move the folder in /Library/Pd or ~/Library/Pd and specify the path in Pd's preferences. Taken that the library has been compiled.
I've compiled it though a couple of times on OS X and had no problems.
