Although someone says they have compiled it against Vanilla in the thread, it might still need to be a 32-bit version given the age of the external and its dependencies.
Pd Extended is still available here.......... http://puredata.info/downloads/pd-extended .... but you might not be able to run it on a recent 64-bit only Mac.

I can run PDl2ork but still can't create the object.

a step by step compiling for dummies would come in handy

But for Linux it should be simple using "make" with the makefile provided.
Have you tried this one?.........https://github.com/tkzic/pd-rtlsdr/tree/master/src/pd/macOS/rtlsdr~
It might not work as the dependencies will most likely be wrong for your system.
Maybe email the author through the GitHub page?...... at the bottom of the "ReadMe"......
